What are the purposes of the 5-step nursing process?
五步護(hù)理程序的目的是什么？
It is to find out the patient's health care needs, to work out a nursing care plan to meet those needs, and to complete nursing interventions to meet those needs.
其目的是發(fā)現(xiàn)患者健康護(hù)理需求、針對這些需求制定護(hù)理計(jì)劃、完成護(hù)理任務(wù)解決這些需求。
To understand its purposes better, let's take a look at each of the 5 steps.
為了更好的理解這一目的，我們來仔細(xì)對每一步驟進(jìn)行理解。
The first step is assessment. At this time, the nurse works together, confirm, and write data about patient in order to establish database.
第一步：評估。這一階段，護(hù)理人員一起確認(rèn)、并記錄患者信息，創(chuàng)建患者病例。
The second step is nursing diagnosis. She tries to discover health care needs of the patients and form nursing diagnosis.
第二部，診斷。護(hù)理人員觀察患者需求，做出診斷。
The third step is planning. Its purpose is to find out the patient's goals, determine priorities of care,
第三步，計(jì)劃。計(jì)劃的目的在于詢問患者的健康目標(biāo)，確定護(hù)理活動優(yōu)先順序，
design the nursing strategies so as to reach goals of care and decide result criteria.
依據(jù)護(hù)理目標(biāo)和測定標(biāo)準(zhǔn)制定護(hù)理策略。
The fourth step is implementation. The nurse tries to complete nursing actions in necessary for accomplishing the plan.
第四步，實(shí)施。護(hù)理人員應(yīng)當(dāng)完成必要的護(hù)理活動，完成護(hù)理計(jì)劃。
Of course, at this stage, the nurse has to reassess patients, review and modify the existing care plan.
當(dāng)然，這一階段，護(hù)理人員應(yīng)當(dāng)對患者進(jìn)行重新評估、并對護(hù)理計(jì)劃進(jìn)行重審和調(diào)整。
The fifth step is evaluation. The nurse should decide how well the nursing goals have been reached.
第五步是評價。護(hù)理人員應(yīng)當(dāng)評價護(hù)理目標(biāo)實(shí)現(xiàn)情況如何。
